Have as much of the following information to hand about the patient as possible.
- Doctors Name
- Patient’s full name
- Date of birth
- Home address (or where they are staying if they are away from home)
- Brief outline of the medical problem
- Any medical history or medicines being taken
Don’t worry if you cannot find this number as the answer machine at the surgery has this recorded on it.
